Injury / Exposure Report:            

Use this form to report any actual OR near-miss injury or exposure that occurs while at work at Life EMS.  Examples of when     this form should be used include:  a back strain resulting from lifting a patient; a needle-stick that occurs while administering a medication; a slip and fall not resulting in an injury but with the potential for injury to occur.  Forms submitted are reviewed by the Director of Health and Safety.
